Sleep problems in menopause

Medically reviewed by Dr. Nathan Roth, MDCM, FRCSC, FACOG · Updated 2026-11-01

Why does menopause disrupt sleep?

Menopause disrupts sleep through night sweats, hormonal effects on sleep architecture, anxiety, and restless legs or sleep apnea that become more common with age and weight change. Treating vasomotor symptoms and using CBT-I principles are first-line strategies. HormoneFit evaluates sleep together with hormone and mood symptoms.

  • Night sweats are a major reversible driver of midlife insomnia
  • CBT-I has strong evidence for chronic insomnia
  • Screen for sleep apnea when snoring, pauses, or refractory fatigue occur
  • Sedatives are not first-line long-term therapy

Assessment

A good history asks about flash frequency, caffeine/alcohol timing, mood, pain, nocturia, and bed-partner observations of snoring. Labs are not required to diagnose menopause-related insomnia but may be used when other disorders are suspected.

Treatment

Improve the sleep environment and consistent schedules. Treat night sweats when present. CBT-I outperforms long-term sleeping pills for chronic insomnia. HRT may help when vasomotor symptoms are the driver; it is not a universal sleep medication.
