Fatigue in perimenopause and menopause

Medically reviewed by Dr. Nathan Roth, MDCM, FRCSC, FACOG · Updated 2026-11-01

Why am I so tired in menopause?

Menopause-related fatigue is usually multifactorial: fragmented sleep from night sweats, mood changes, lower activity, and sometimes iron deficiency from heavy bleeding or coexisting thyroid disease. Treating the drivers — especially sleep and vasomotor symptoms — helps more than caffeine alone. HormoneFit evaluates fatigue in context rather than attributing everything to “just hormones.”

  • Night sweats and insomnia are top reversible causes
  • Heavy periods can cause iron-deficiency fatigue
  • Check thyroid and mood when fatigue is profound
  • HRT may help when flashes destroy sleep — it is not a stimulant

A practical workup mindset

Ask about sleep quality, flash frequency, bleeding volume, snoring, depression, medications, and exercise. Bloodwork is guided by history — not a fixed menopause panel for every tired woman.

If fatigue is sudden, associated with chest pain, breathlessness, or neurological change, seek urgent care rather than routine menopause booking.

What helps

Restore sleep by treating night sweats when present. Address iron deficiency. Build gradual activity and protein intake. Consider CBT for insomnia. Discuss HRT or non-hormonal flash therapy when vasomotor symptoms are severe.
