Red flags
- Changing pads or tampons every hour for several hours
- Passing large clots
- Bleeding after sex or between periods
- Dizziness, shortness of breath, or known low hemoglobin
- Bleeding after menopause (12 months without a period)
Evaluation
History, pelvic exam, pregnancy test when relevant, CBC/ferritin, and pelvic ultrasound are common first steps. Endometrial sampling may be required based on age and risk factors before hormonal treatment for bleeding.
Treatment
Options include tranexamic acid, NSAIDs, hormonal contraceptives, progestins, LNG-IUS, and procedural care when structural lesions exist. Once bleeding is safe and controlled, broader menopause symptom management can proceed.
