Does progesterone cause weight gain?

Medically reviewed by Dr. Nathan Roth, MDCM, FRCSC, FACOG · Updated 2026-11-01

Does progesterone in HRT cause weight gain?

Some women notice fluid retention, breast tenderness, or appetite change on progestogens, but micronized progesterone is not proven to cause large fat-mass gain for most users. Midlife weight change is driven mainly by aging, muscle loss, sleep disruption, and central fat redistribution after estrogen decline. If a specific progestogen feels intolerable, regimens can often be adjusted. HormoneFit separates medication side effects from menopause metabolism myths.

  • Anecdote ≠ guaranteed fat gain
  • Sleep and muscle loss matter more than progesterone alone
  • Different progestogens feel different — report side effects

If you feel bloated on progesterone

Timing (night dosing), dose, and continuous vs cyclic schedules can be revisited. Do not stop endometrial protection without a clinician if you still have a uterus and take systemic estrogen.
