Headaches and migraine in menopause

Medically reviewed by Dr. Nathan Roth, MDCM, FRCSC, FACOG · Updated 2026-11-01

Can menopause cause headaches?

Yes. Hormone fluctuation can change migraine pattern — some worsen in perimenopause, some improve after menopause. New severe headache, neurological deficits, or thunderclap onset need emergency evaluation. Migraine with aura modifies HRT counseling; HormoneFit documents aura carefully before considering estrogen.

  • Pattern change in the 40s is common
  • Aura history changes estrogen risk discussion
  • Transdermal estradiol is often preferred if HRT is used
  • Red-flag headaches are not “just menopause”

Care approach

Track frequency, aura, triggers, and blood pressure. Optimize migraine prevention. See our migraine-with-aura HRT article before assuming estrogen is banned or automatically safe.
