How soon does HRT work for hot flashes?

Medically reviewed by Dr. Nathan Roth, MDCM, FRCSC, FACOG · Updated 2026-11-01

How soon can HRT start working for hot flashes?

Many women notice fewer or milder hot flashes within a few weeks of starting systemic HRT, with fuller benefit over one to three months as dose is optimized. Vaginal estrogen for GSM improves dryness over weeks of consistent use. If nothing changes after an adequate trial, regimens are adjusted or alternatives considered. HormoneFit sets expectations and follow-up rather than “set and forget.”

  • Partial improvement often by 2–4 weeks
  • Dose titration may be needed
  • GSM local therapy has its own timeline
  • Non-response deserves reassessment, not blame

What to track

Flash frequency, night awakenings, and side effects (breast tenderness, spotting). Bring a simple symptom log to follow-up.
