Does HRT increase stroke risk?

Medically reviewed by Dr. Nathan Roth, MDCM, FRCSC, FACOG · Updated 2026-11-01

Does hormone replacement therapy increase stroke risk?

Stroke risk with systemic HRT depends strongly on age, time since menopause, dose, and route. Oral estrogen can raise clot and stroke risk more than transdermal estradiol in observational analyses. For healthy women who start HRT near menopause for bothersome symptoms, absolute stroke risk increases are often small and should be discussed with personal risk factors (blood pressure, smoking, migraine with aura). HormoneFit prefers individualized regimens and often considers transdermal estradiol when estrogen is appropriate.

  • Timing and route matter more than a yes/no scare headline
  • Transdermal estradiol may have a more favourable clot profile than oral estrogen
  • Control blood pressure and stop smoking
  • Migraine with aura changes counseling

Putting risk in perspective

Media coverage often quotes relative risk without absolute numbers. For many newly menopausal women, the absolute increase in stroke events attributable to appropriately prescribed HRT is modest compared with the relief of severe vasomotor symptoms — but it is not zero, and higher-risk patients need different plans.

Practical steps at HormoneFit

We document cardiovascular history, migraine aura, smoking, and blood pressure; discuss oral vs transdermal options; and revisit therapy if new neurological symptoms appear. Emergency care comes first for sudden weakness, speech change, or worst-ever headache.
