Does HRT cause cancer?

Does HRT cause cancer?

Hormone therapy does not simply “cause cancer” for every user. Breast cancer risk with combined estrogen-progestogen therapy is a small absolute increase for some women with longer use, while estrogen-alone therapy after hysterectomy has a different risk profile. Decisions depend on personal/family history, age, regimen, and duration. HormoneFit clinicians in Ontario individualize this discussion before prescribing.

  • Risk is nuanced — not a yes/no for all women and all regimens
  • Combined HRT may slightly raise breast cancer risk with longer use for some
  • Estrogen-alone after hysterectomy has a different evidence profile
  • Personal and family cancer history must guide decisions
  • Benefits for severe symptoms often outweigh small absolute risks when started appropriately

What the evidence actually shows

Public fear of HRT rose after early WHI reporting. Later analyses and modern lower-dose regimens show a more nuanced picture: absolute risk increases for breast cancer with combined therapy are typically small for many midlife women, especially with shorter or carefully monitored use, while untreated severe symptoms also carry quality-of-life and sometimes health costs.

Who needs extra caution

Personal history of hormone-sensitive breast cancer, certain genetic risks, and some unexplained bleeding situations require specialist-aligned decisions. Prior cancer does not always mean “never any hormone” — local vaginal estrogen may still be discussed in select cases with oncology input.
